Book your appointment with your health coach

Meet with one of our specialty trained health coaches for your first 30 minute intake appointment.  The goal during this 30 minute phone call is to dig into your medical history, symptoms and goals. Based on this information, we will order lab work to be completed before your appointment with your clinician. Your first appointment with your clinician will also be booked during this call. Essentially, this appointment expedites your healing journey.

Check your confirmation email for next steps

You will receive a detailed confirmation email that provides step-by-step instructions on how to create a MTC patient portal and complete our new patient paperwork. These steps are also included below for your convenience.

• Click the button below to fill out our registration page. Once completed, you will receive an email that will allow you to sign-in to our portal system and move on to the next step.

• Once your registration form is completed, you will receive an email that will direct you to your patient portal. You will fill out the new patient paperwork in the QUESTIONNAIRE section of the portal.

• If you would like to add any past labs or documents for your clinician to review, you can do so in the Lab and Document area of the patient portal

Expect a call from us 48 hours before your appointment

You will receive a call from Bre Higgins, our wonderful patient coordinator, to confirm that all your new patient paperwork is complete and to address any questions you might have about your call with your health coach or any other part aspects of your care at MTC.
